Camper Forms to be Completed by Parents

Please print and fill out the forms below, and return them to the Camp Funtime Office. All forms need to be turned into the Camp Office by June 1, 2012. If you register after June 1st, forms will be due at time of registration.

Camper Medical Form – Mandatory for all campers, please fill out one copy. Each camper must have their own.

Field Trip Medical Form – If your camper will be attending any of the field trips, please fill out one copy of this form.

Camper Pick-Up Form – Mandatory for all campers. We need one copy for each camper. Families with more than one camper may use one form for all campers.

Camper Prescription Medication Form – Optional. If your child will be taking prescription medication while attending Camp Funtime, please print out this form and have it filled out by your child’s pediatrician. We only need one copy of this form.
